Family Medical & Dental Centers 


Rural Health Care, Inc. d/b/a Family Medical & Dental Centers (FMDC) is a private non-profit community and migrant health center established in rural Putnam County, Florida in 1977.  FMDC is also a federally qualified health center (FQHC) and operates in areas designated as Health Professional Shortage Areas and or Medically Underserved Areas or Populations.  FMDC physicians are Board Certified or eligible in Family Practice, Internal Medicine and/or Pediatrics.

Since its founding in 1977 Family Medical & Dental Centers has grown to include locations in 4 counties in northeast Florida.  Today, FMDC operates 8 full-time medical centers, 2 full-time dental centers and 5 full-service pharmacies in 4 contiguous counties in Florida and is also a major employer with more than 170 full-time employees.

Family Medical & Dental Centers is governed by a 13-member Board of Governors that meets each month to discuss various aspects of providing the best care and best service to our patients.
